IMPLEMENTASI PELAYANAN KEFARMASIAN DI INSTALASI FARMASI RUMAH SAKIT ANGKATAN DARAT ROBERT WOLTER MONGISIDI MANADO.

ABSTRAK

Pelayanan Farmasi Rumah Sakit merupakan salah satu kegiatan di rumah sakit yang menunjang pelayanan kesehatan yang bermutu. Berdasarkan peraturan menteri kesehatan no 58 tahun 2014 tentang standar pelayanan kefarmasian di rumah sakit di Indonesia sehingga perlu dilakukan penelitian penerapan pelayanan kefarmasian di instalasi farmasi RSAD Robert Wolter Mongisidi Manado. Mengingat pentingnya implementasi pelayanan kefarmasian obat yang dimulai dari pemilihan, perencanaan kebutuhan, pengadaan, penerimaan, penyimpanan, penditribusian, pemusnahan dan penarikan, pengendalian dan administrasi dalam persediaan obat di rumah sakit.

Penelitian ini dilakukan dengan menggunakan metode kualitatif yang bertujuan untuk mendapatkan informasi yang lebih mendalam tentang bagaimana penerapan pelayanan kefarmasian obat yang dikeluarkan oleh pemerintah di IFRSAD R.W. Mongisidi Manado. Dalam menetapkan responden menggunakan teknik Snowball sampling, melibatkan 7 orang responden. Data primer diperoleh melalui wawancara mendalam dan observasi  check list dari wakil kepala RSAD, Kepala instalasi farmasi RSAD, Kepala Tata Usaha, bagian pelayanan, bagian perencanaan dan gudang. Data sekunder diperoleh dari form-form checklist observasi pelayanan kefarmasian meliputi pemilihan, perencanaan kebutuhan, pengadaan, penerimaan, penyimpanan, pendistribusian, pemusnahan, pengendalian dan administrasi obat di IFRSAD R.W. Mongisidi Manado.

Hasil penelitian menunjukkan RSAD dalam pemilihan belum membentuk Tim Farmasi dan Terapi, dan belum menyusun formularium obat, diperoleh pemilihan (2,60%), perencanaan kebutuhan (3,90%) berdasarkan metode konsumsi, pengadaan obat (6,49%) dibeli secara langsung di PBF dan ada obat dropping dari Kesdam dan Pusat, penerimaan (15,58%) tidak ada panitia khusus, penyimpanan (14,29%) obat secara FIFO dan FEFO, pendistribusian (3,90%) obat untuk rawat jalan secara individu dan untuk rawat inap menggunakan metode kombinasi, pemusnahan dan penarikan obat (1,30%) yang sudah kadaluwarsa dan rusak tidak pernah dilakukan, pengendalian (3,90%) belum sesuai dengan standar,   administrasi (19,48%) dalam hal pencatatan dan pelaporan belum berjalan optimal karena kurangnya pengawasan dan evaluasi dari manajemen rumah sakit. Hasil observasi total nilai diperoleh 72,73% digolongkan sedang.

Penelitian ini dapat disimpulkan implementasi penerapan pelayanan kefarmasian yang dilakukan di IFRSAD R.W. Mongisidi Manado belum sesuai dengan standar pelayanan kefarmasian di rumah sakit yang ditetapkan dalam Peraturan Menteri Kesehatan Nomor 58 Tahun 2014. Saran yang diajukan harus membentuk Tim Farmasi dan Terapi dan menyusun formularium obat, dan  membuat standar prosedur operasioanal (SPO) serta melakukan perbaikan dan peningkatan pelayanan kefarmasian sesuai dengan standar pelayanan kefarmasian di rumah sakit yang ditetapkan dalam Peraturan Menteri Kesehatan Nomor 58 Tahun 2014.

ABSTRACT
Hospital Pharmacy Services is one of the activities in hospitals that support quality health services. Under the ministerial decree No. 58 of 2014 health standards pharmacy services in hospitals in Indonesia so it is necessary to study the application of pharmacy services in pharmacy RSAD Robert Wolter Mongisidi Manado. Given the importance of the implementation of pharmaceutical services starting from the selection of drugs, demand planning, procurement, receipt, storage, distribution, extermination and withdrawal, control and administration of the drug supply in hospitals.


                     This research was conducted using qualitative methods aiming to obtain a more in-depth information about how the application of the drug pharmacy services issued by the government in IFRSAD RW Mongisidi Manado. In setting the respondents using the Snowball sampling techniques, involving seven respondents. The primary data obtained through interviews and observation check list of RSAD of the deputy head, head of pharmacy RSAD, Head of Administration, part service, part of the planning and warehouse. Secondary data were obtained from the observation checklist forms pharmacy services include the selection, demand planning, procurement, receipt, storage, distribution, destruction, control and administration of the drug in IFRSAD RW Monginsidi Manado.
                    The results showed RSAD in recent elections form a team of Pharmacy and Therapeutics, and yet arrange drug formulary, obtained election (2.60%), demand planning (3.90%) based on the method of consumption, drug procurement (6.49%) purchased directly in PBF and there are drugs and dropping out of Kesdam Center, acceptance (15.58%) there is no special committee, storage (14.29%) drug FIFO and FEFO, distribution (3.90%) for outpatient drugs individually and for inpatient use a combination of methods, culling and drug withdrawal (1.30%) that have expired and damaged never carried out, the control (3.90%) is not in accordance with the standards, the administration (19.48%) in terms of recording and reporting is not optimal due to the lack of monitoring and evaluation of hospital management. The results of observations obtained 72.73% total value being classified.
                    This study we can conclude the implementation of the application of pharmacy services conducted in IFRSAD RW Manado Monginsidi not in accordance with the standards of hospital pharmacy services set out in the Minister of Health Regulation No. 58 Year 2014. The suggestions put forward must form teams of Pharmacy and Therapeutics and develop drug formulary , and sets Standards Operational procedures (SOP) and perform repairs and service improvement pharmacy in accordance with the standards of hospital pharmacy services set out in the Minister of Health Regulation No. 58 Year 2014 .
